Write down changes in memory, routines, communication, mobility, sleep, or safety. Bring questions and observations to the person’s clinical team.

Protect familiar routines

Plan support around preferred meals, personal care, activities, language, music, and the times of day that feel most difficult.

Plan the caregiver handoff

Identify who needs respite, what support is authorized, which tasks matter most, and how updates should be shared after a visit.

Review the home environment

Look together at lighting, walkways, bathroom safety, medication storage, doors, and emergency contacts without making unsupported clinical assumptions.
