Use this checklist as a starting point for a respectful, practical conversation. It is not medical advice.

Daily rhythm and preferences

  • Preferred name, language, and communication style
  • Typical meal, sleep, personal-care, and activity routines
  • Comfort cues, favorite topics, music, or familiar objects

Safety and mobility conversation

  • Mobility changes, fall concerns, and helpful equipment
  • Lighting, walkways, bathroom, kitchen, and door-safety concerns
  • Emergency contacts and the clinician or care team to call with questions

Caregiver handoff

  • Support the family caregiver needs during a planned break
  • Tasks the caregiver is authorized and expected to help with
  • What should be documented or shared after a visit

Questions for the first care conversation

  • Which times of day need the most cueing, companionship, or practical help?
  • What would make the first visit feel familiar and respectful?
  • What timing, schedule, or coverage questions should be reviewed with Koi?
